Eating & drinking
Can You Drink Coffee or Water Before Surgery?
Short answer: usually yes — water and even black coffee are allowed up to about 2 hours before your hospital arrival time. Here's what counts as a clear liquid, and what to avoid.
The 2-hour rule
What counts as a clear liquid?
A clear liquid is simply one you can see through. These leave your stomach quickly — in about two hours — which is why they're treated differently from food and milk.
| Allowed (clear) | Not allowed |
|---|---|
| Water | Milk, cream, or whitener |
| Black coffee or tea (no milk) | Lattes, cappuccinos |
| Apple juice & pulp-free juice | Orange juice with pulp |
| Clear (see-through) sports drinks* | Smoothies & milkshakes |
| Anything you can clearly see through | Cloudy, colored, or pulpy drinks |
*Some hospitals limit drinks to plain water and clear juice, and exclude broth or sports drinks. When in doubt, stick to water — and follow your hospital's list.
Coffee: the rules
Coffee is fine only if it's black. The moment you add milk, cream, or a dairy whitener, it stops being a clear liquid and is treated like food. Sugar and artificial sweeteners are okay. A single black coffee on the morning of surgery is a common allowance — but confirm the timing with your team.
Important
Why the rule exists
Under anesthesia, the reflexes that normally keep food and liquid out of your lungs are switched off. If your stomach isn't empty, its contents can come up and be breathed into the lungs — a dangerous complication called aspiration. Clear liquids empty fast, so a small amount up to 2 hours before is considered safe. Read the full reason here.

Frequently asked questions
Can I drink water before surgery?
Yes. Water is a clear liquid and is usually allowed up to 2 hours before your hospital arrival time, unless your surgical team tells you otherwise.
Can I drink coffee before surgery?
Black coffee with no milk or cream is a clear liquid and is generally allowed up to 2 hours before your hospital arrival time. You can add sugar or sweetener, but not milk, cream, or a milk-based whitener.
How many hours before surgery should I stop drinking?
For most adults, stop all clear liquids 2 hours before your hospital arrival time, and stop solid food and milk 8 hours before your hospital arrival time. Always follow your team's specific instructions.
Does milk or a latte count as a clear liquid?
No. Milk, cream, lattes, and smoothies are treated like food because they take longer to leave the stomach. Only liquids you can see through count as clear liquids.
