Fasting Before a Tonsillectomy

Tonsillectomy is done under general anesthesia and is common in children, so fasting — including the special milk timings for little ones — matters.

Medicines to check

  • Blood thinners (aspirin, clopidogrel, warfarin, Eliquis, Xarelto) — see medications to stop.
  • Diabetes medicines & insulin — they need a plan. See diabetes tablets.
  • GLP-1 (Ozempic, Wegovy, Mounjaro) — tell your team. See the GLP-1 guide.
  • Blood pressure medicines — confirm which to take with a sip of water and which to hold.
  • Avoid ibuprofen/aspirin-type painkillers before surgery unless your team approves (bleeding risk).

When this surgery may be delayed

  • Fever, a new cough/cold, or a chest infection
  • Vomiting or diarrhea
  • Very high blood pressure or blood sugar
  • You ate or drank outside your fasting window
  • An active throat or chest infection or fever

Reports & documents to carry

  • Photo ID and insurance card

What to ask your anesthesia team

  • Will I have general anesthesia, spinal/regional, or sedation?
  • Which medicines do I take or hold on the morning of surgery?
  • When can I eat and drink afterward?
  • Will I go home the same day, and who can drive me?
  • For my child: exactly when do breast milk and formula need to stop?

Frequently asked questions

How long should my child fast before a tonsillectomy?

Usually solids about 8 hours before, breast milk about 4 hours before, formula or cow's milk about 6 hours before, and clear liquids about 2 hours before your hospital arrival time. Use the calculator's child option for exact times.

Can my child drink water before a tonsillectomy?

Yes — small amounts of clear liquid (water, clear juice) are usually allowed up to about 2 hours before arrival, which helps a child feel less hungry and upset.
